Pay Student Athletics Fee
Each student participating in athletics must pay a one-time annual fee. If your child plays one sport during the school year, the total fee is $75. Two sports cost $125. And, if your child plays three sports, it is $150. The check should be made payable to St. Dennis Athletics and sent into the office or delivered directly to the Athletic Director.
Submit Athletic Permit Form
This form includes inquiries for basic contact information, healthcare and insurance information and grants permission for your child to participate in athletics. The form can be filled out online and submitted directly to the athletic director, or you can print it and return it with your fee payment.
Athletic Handbook Acknowledgement Form
Parents must read and understand the St. Dennis Athletic Handbook, which includes information about our mission, volunteer requirements, rules, MAISL bylaws and academic requirements for athletic eligibility. Once read, sign the acknowledgement form and return to the school.
